A form which must be filed by the insurance company stating that auto liability insurance is in effect for a particular individual is what is commonly called as the sr22 or the financila responsibility certificate. THis is required when insurance is provided to a person who got involved in an accident or was convicted of a moving violation and was unable to show proof financial responsibility.
